The initial stage of labor is contraction which causes cervical thinning and opening which consists of two phases. The first is the latent phase where cervical opening occurs from 1cm to 3cm. The second is the active phase marked by cervical opening from 4 cm to 10 cm (complete). The latent phase has a longer duration than the active phase.

In the first pregnancy or normal latent phase primigravida can occur up to 12 hours. The duration of the latent phase is affected by the condition of the cervix, and fake contractions
analgesic or anesthetic use. Then the active phase can last for 1-2 hours. In primigravida, long labor is categorized if the progress of cervical opening in the active phase <1.2 cm / hour.

Here are the factors causing prolonged labor:
1. Inefficient contraction
2. Fetal factors (malpresentation, malposition, large fetus)
3. Birth canal factors (narrow pelvis, cervical, vaginal, tumor abnormalities)
