Bilirubin Blood Test?
Hello my dad had a stage 1 Kidney cancer in 2015 and this year on a check up test they found metastatic nodules on his lung … the doctor asked for some tests including Ca 19.9 Ca 125 CEA Lactate dehydrogenase GGT ALT AST and Bilirubin every test are normal beside direct bilirubin that checked at 0.31 (0.2 reference) should i be worry about liver metastasis?

Kidney cancer is the disease in which abnormal cell growth is found on the kidney cells. Renal cell carcinoma is the most common type of kidney cancer found in adults. Most of kidney cancer is found before theses abnormal cells spread (metastasize) to other organs. However, they are usually detected when the size is already big. There are several stages of kidney cancer, from stage I to stage IV. Soemone is said to have stage I if the size of tumor is 7 cm or smaller, confined to the kidney. If there is a spread to other organs, it is classified as stage IV. Kidney cancer can spread to the orthe kidney, lymph nodes around the kidney, kidney blood vessel, adrenal glands (glands above the kidneys), lungs, liver, bones, and brain.
THe first investigation to detecet any spread to the liver is by doing blood test on liver function including AST (aspartate amino transferase), ALT (alanine aminotransferase), GGT (gamma glutamyl transferase), and bilirubin level. Usually liver function test will show abnormal result (high result) if there is metastatic tumor. Direct bilirubin in your father's case increased insignificantly and this increased result can be due to other numerous causes as the other findings were normal. However, with your father's medical history, it is best for you to consult this finding to his doctor. The doctor might do further investigation including ultrasound or CT scan to detect any liver metastasis as that is the only way to confirm it.
