Large intrabdominal pressure (in the stomach) can cause the navel to appear flat, this can be observed in large pregnant women, patients with ascites (abnormal fluid in the stomach), or the presence of very large tumors in the stomach. Navel that is so prominent (bulging) that looks like a lump can be caused by an umbilical hernia. An umbilic hernia occurs due to a hole in the abdominal wall muscles, which results in part of the intestines poking out when there is increased intraabdominal pressure including when coughing, sneezing and straining.
In your father's case an enlarged abdomen accompanied by swelling in the lower limbs can be caused due to edema associated with fluid retention. Most of the body consists of fluid, this fluid is mostly located in blood vessels and partly compiles components in body cells. Edema can occur when fluid from a blood vessel comes out and occupies a space between body tissues. Edema can occur in all parts of the body, but the most easily observed are in the back of the foot, ankle, calf, eyelid. Edema can also occur in internal organs such as the lungs, brain, kidneys, and so on. While the term used for abnormal fluid collection in the abdominal cavity is ascites.
